All 50 U.S. States to Receive $50B for Rural Health Care

Trump administration announces unprecedented five-year investment under the Rural Health Transformation Program to modernize facilities, expand workforce, and improve access in rural America.

The Trump administration has announced a historic $50 billion federal investment aimed at strengthening rural health care across all 50 U.S. states, marking the largest-ever national funding initiative dedicated to rural medical services.
According to a White House statement released on December 29, 2025, the funding will be distributed through the Rural Health Transformation Program (RHTP), a major initiative established under the Working Families Tax Cuts legislation. The program will allocate $10 billion annually over five fiscal years, from 2026 through 2030.
The initiative seeks to address long-standing disparities in rural health care by expanding the rural health workforce, modernizing medical facilities and technology, and introducing innovative care delivery models to ensure quality health services regardless of geographic location.
“Access to quality care should not be determined by your zip code,” the White House said, emphasizing that the program is designed to bring high-quality health services closer to rural communities nationwide.
Each state will receive individual funding awards based on program criteria, with larger allocations going to states with extensive rural populations. Texas is set to receive the highest annual award at approximately $281.3 million, followed by Alaska ($272.1 million) and Montana ($233.5 million). Other major recipients include California, New York, Florida, and Pennsylvania, each receiving over $190 million.
The administration highlighted that the initiative was passed without Democratic support, noting that every Democratic lawmaker voted against the legislation that enabled the funding.
Health policy experts say the investment could significantly improve rural hospital sustainability, telehealth expansion, and emergency care access, particularly in underserved and geographically isolated regions.
The Rural Health Transformation Program is expected to play a key role in reshaping America’s rural health infrastructure over the next decade amid rising concerns about hospital closures, workforce shortages, and unequal access to care.
